Initial consultation with Colleen Tetzlaff, DNP, FNP, 150.00 for 30” in office. All interested potential patients will be required to secure an initial consultation to determine best treatment plan. Following the initial consultation, monthly follow up appointments at 20” per session will be required the first 3 months at 75.00 per visit in office for those using any pharmaceuticals or weight management kits to support weight loss. After the first 3 months of visits, the frequency of follow ups and use of telehealth as appropriate will be determined.
Nutrition and health coaching with Mary Moskowitz $400.00 for 3 months of services. Includes a one-hour initial consultation via ZOOM with practical information provided for dietary guidance, followed by biweekly ZOOM check-in 30” sessions x5. Required for all patients seeking help with weight loss and may also be recommended for others seeking care for chronic disease conditions.
Injectables
Semaglutide treatment $50-100.00 per month depending upon dosing. A monthly check in with Colleen Tetzlaff also required for ongoing prescribing at 75.00 a session 20” check in (as noted above health and nutrition counseling required first 3 months on Semaglutide)
Lipo mino mix $65.00 per month. A popular weekly injectable to help with fat burning energy production and metabolism support.
B12 shots $25.00 a shot.
Weight loss management kits $225.00 per month, utilizing 3 medicinals: low dose naltrexone capsules, Semorelin as a subcutaneous injectable and Lipo-trim spray to support metabolism and energy production, as well as fat burning and sleep. May be combined with Semaglutide or on its own within a comprehensive program of diet, exercise and lifestyle modifications.
IV therapies
Weight loss IV B-Lean $110.00. Suggested use every 2-4 weeks alongside any of the above and a diet and exercise program. Designed to help with fat burning and boosting of metabolism.
Myers cocktail IV $165.00. A classic formula of vitamins and minerals for health and wellness, especially helpful to support fatigue issues and overall health.
Other IV formulas for hair, skin and nail support as well as immunity are also available upon request.
